Religious Education Registration Form - 2024

A copy of the following documents are required:
Photo
Birth certificate
Baptism certificate
First Holy Communion certificate.

If the parent is a registered member of the parish, then a copy of the envelope (or envelope number) is required for processing the application. If not registered, then the family will be registered with the parish upon the submission of the application of their child for religious education.

For the year 2024-25, the registration fees are as follows:
1 Child - $150
2 Children - $240
3 Children - $350

The payment could be done online while submitting the form or in-person at the parish office.

NEW STUDENT:
Is a child/adult who is registering for the first year of the religious education program.

RETURNING STUDENT:
Is a child/adult who attended the first year of the classes and now is returning for the second year of the religious education program.

For example:
If your child is 7th grade, not done his first holy communion and registering for the first year then you choose - "NEW STUDENT: First Holy Communion (3-7 Grade)"

If your child is 10th grade, not done his confirmation and registering for the first year then you choose - "NEW STUDENT: Confirmation (9-12 Grade)"

If your child is 4th grade, not done his first holy communion and registering for the second year then you choose - "RETURNING STUDENT: First Holy Communion (3-7 Grade)"

Sunday Mass Participation for First Holy Communion Students (Grades 3-7)

I, the undersigned, understand that my child/children MUST attend Mass every Sunday, (Saturday 5pm+7pm Mass also count).  He/She will have the Sunday bulletin or another “official” sheet signed by the celebrating Priest, Deacon, Lector, Eucharistic Minister, Catechist, and or Usher AFTER Mass. I understand this is a Homework assignment that must be handed in weekly.  Failure to submit this weekly assignment, may result in my child/children NOT receiving credit for the year and or NOT receiving the sacrament they are preparing for.

Sunday Mass Participation for Confirmation Students (Grades 9-12)

I, the undersigned, understand that my child/children MUST attend Mass every Sunday at 11am as part of the sunday classes. The student/parent is not to request his catechist to leave before mass, unless this has been discussed days ahead with the Confirmation Co-ordinator or it is an emegency. Failure to attend mass will result in absence for that class and under extreme conditions NOT receiving the sacrament they are preparing for.

Waiver for Pictures/Photos/Video Permission and Release

I, hereby grant permission, without reservation, to those authorized by Our Lady of Loretto Church, its employees, or agents the right to take photographs, videotape or digital recordings of my child(ren) on this form and to use them in original or modified form in all media now or hereafter known including, without limitation, websites, bulletins, newsletters and promotional brochures with or without name of information solely for the promotion, public education and/or fundraising activities of Our Lady of Loretto Church. I understand and agree that I am entitled to receive no compensation for the above. I release Our Lady of Loretto Church and assignees its officers, directors, agents, employees, independent contractors, licensees, and assignees from all claims that I now have or in the future may have relating to the above. Class Absence Policy

I understand the absence policy for Our Lady of Loretto Religious Education only allows 5 absences. Each absence must be accompanied by a Doctors note upon the students return to class. If my child is absent any further, they will have to restart the program in the following year. I understand the registration fee paid for this term is non-refundable.

Entiendo que el programa de Catequesis de Nuestra Sra. De Loretto solo le permite a mi hijo(a) 5 ausencias por año. Cada ausencia deberá ser acompañada con una nota del Doctor confirmando que el niño estuvo enfermo. Si mi hijo(a) falta más de eso tendrá que retomar las clases el próximo año. Por lo mismo entiendo que no habrá rembolso del dinero que se pagó por la matriculación.

Regristration fee could be paid online or in-person at the parish office. If you like to pay cash in-person, please choose the "WILL PAY IN CASH" option from the dropdown. If you are paying online, please choose the amount correctly based on the number of children you are registering.
